University Senate forms for courses that require separate Senate C&C approvalForms to file with Registrar or Graduate School once CLAS Committee on Curricula & Courses has approved your proposals
- Certain course proposals must be approved by the University Senate, including: courses at 100's level; courses open to sophomores; courses with skill level W, Q, and/or C; and General Education courses. In such cases two proposals are required, one to CLAS CC&C (using our forms) and one to the Senate CC&C (using their forms).
- Once a course has been approved, it is the responsibility of the department to complete these forms and submit copy to the Registrar or Graduate School, including dates of all approving minutes. Your Department representative should tell you who is responsible for filling out and checking these forms before they are filed.

NOTE: there is no form to change an existing major. Send a memo or e-mail from the dept. head to the undergraduate catalog editor, Marianne Buck, with the revised major description as approved by CC&C- Graduate School Course Forms page : for graduate courses.
